On-call Work Needs A Narrow Playbook
An on-call AI agent workflow is useful only if it reduces decision fatigue. If every alert turns into a fresh judgment call, the agent is just another source of pager noise. If every alert gets ignored until morning, the agent loses most of its value.
The right model is narrower: decide ahead of time which requests you can handle from a browser or phone, which requests should pause, and which ones should be escalated to desktop review. That is the same discipline you would use for production incident response. The difference is that the work is happening in a local repo through Claude Code or Codex rather than in a cloud sandbox.

Define What On-Call Means

The class that should not be on-call is the one that requires deep context and long reading. Schema changes, security-sensitive logic, broad dependency upgrades, and anything touching shared state should not be approved just because a notification arrived on your phone.

Build A Response Ladder
The easiest way to make on-call sustainable is to define a response ladder before the first alert arrives.

Make Notifications Decision-Shaped
Notifications are only useful when they point to a decision. A message that says "something changed" is not enough. A message that says "approval needed for a package test" is much better.

Suppose a Claude Code session is fixing a failing test in a web package. The run asks to execute the package test command, and the diff only touches that package.
That is a good on-call approval if all of the following are true:
- the repo is the one you expected
- the branch matches the task
- the command is limited to the package under repair
- the agent already showed why the test was needed
If the same run starts asking for a database migration or a repo-wide refactor, the approval changes shape immediately. Stop the run, restate the task, and move the rest of the work to a better review window.
The point is not to approve more. The point is to approve the right amount.
Keep A Short On-Call Policy
You do not need a long handbook. You need a few rules that a tired person can follow.
From mobile, approve only scoped edits, known validation commands, and PR creation for low-risk work.
Do not approve migrations, destructive commands, broad dependency changes, or tasks that touch shared state.
If the request no longer matches the original task, stop and re-evaluate.That policy is small enough to remember and strict enough to prevent most bad decisions.
Tradeoffs Worth Accepting
An on-call workflow always gives up some autonomy. That is deliberate. A run that pauses for a human check is not failing; it is obeying the control model.
There is also a boundary problem. If every small change can wake you up, the workflow becomes noise. The fix is not more alerts. The fix is a narrower approval policy and better task shaping.
The best teams accept that not every alert deserves immediate action. They use the control surface to protect focus as much as to keep work moving.
